Immigration Department

ID 678 authorizes another person to collect a Hong Kong identity card or a specified travel document.

  • ID 678 completed in block letters with a black or blue pen and signed.
  • The representative’s valid original ID; the representative must be at least 18.
  • ROP 140 for an identity card, or the Collection Notice or receipt for a travel document.
  • Any old document and original supporting records named in the notice.
  • Image guide—child’s China Travel Document: prepare the pickup form, a letter of authorization, the child’s valid original document (for example a travel document, passport, or birth certificate), and a copy of the guardian’s valid ID (for example an identity card, travel document, or passport). The guardian’s name and document number in the letter must match the ID copy; the guardian must hand-sign it, not have the representative sign, and state the representative’s name, the child’s name, and the date.
  • Image guide—Re-entry Permit: prepare the collection receipt and the applicant’s valid original identity/travel document listed on the receipt (for example a Hong Kong identity card, passport, or Re-entry Permit). Follow the receipt for the collection office and time. These two items describe the service-centre process shown in the images; they do not replace ID 678, and any service-centre or receipt requirement controls.
  • Image guide—identity card: the image marks the fields to complete in red and asks for the identity-card application receipt; the official form itself requires block letters in black or blue ink and deletion of inapplicable options.
  1. Enter the authorizer, representative, and document details; delete options that do not apply.
  2. Use the same authorizer signature as the original application.
  3. Check the collection date, office, and required records on the notice.
  4. The representative submits ID 678, personal ID, and collection record, then signs for receipt.
  5. Official limitation: if a passport application was submitted by post, drop-in, or online, or a Document of Identity/Re-entry Permit application was submitted by post or drop-in, the applicant may have to collect in person and produce the original supporting documents submitted with the application. For an authorized travel-document collection, the representative must also bring the collection notice/receipt and the old travel document, if any, for cancellation.
