Using the library

This site publishes official authorization letters, blank reference PDFs, and collection guides. It does not accept completed forms.

Open the authorization letter or reference PDF from the relevant institution guide. Check the document title, version, page count, and signature fields. Print it when the form requires a hard copy or handwritten signature.

Complete the institution’s application, payment, or appointment first. Then prepare the authorization form, the applicant’s ID copy, the representative’s original ID, and the collection notice. Keep completed files off public drives and this site.

Maintainers can edit the three Markdown language collections and four PDF folders in Pages CMS. When replacing a form, update the relevant guide in all languages, its official links, and its reference form. One site stylesheet controls the visual design.
